Resource Assistance
Advocates assess each individual’s needs and make referrals to appropriate resources in the community.
Services
Trained Advocates visit the home of participants to determine eligible programs and services such as:
The program provides free health insurance counseling for people with Medicare and their families or caregivers. Advocates are trained MMAP counselors that provide education, assistance and advocacy related to Medicare and Medicaid.
- Medicare/Medicaid Applications
- Prescription Drug Coverage
- Medicare Health Plans
- Medicare/Medicaid Fraud and Abuse
- New Health Care Options
Department of Health and Human Services Programs (DHHS)
Advocates may identify which DHHS programs participants qualify for and assist with the application process. Application assistance may include the following programs:
- Food Assistance (SNAP)
- Emergency Relief (Utility, Burial, and Home)
- Medicaid
- Subsidies to Assist with Medicare Premiums
- MI-Bridge Application Completion
Information and Referral
Information and referrals of a wide range of services are provided to help older adults and their families. Potential resources may include:
- Macomb County Government Services
- Caregiver Assistance
- Housing Resources
- Transportation Resources
- Elder Abuse Services
- Legal Assistance
Eligibility
- Macomb County resident.
- Age 60 and older.
